Суплотова Л.А., Авдеева В.А., Пигарова Е.А., Рожинская Л.Я., 2019</copyright-statement><copyright-year>2019</copyright-year><copyright-holder xml:lang="ru">Суплотова Л.А., Авдеева В.А., Пигарова Е.А., Рожинская Л.Я.</copyright-holder><copyright-holder xml:lang="en">Suplotova L.A., Avdeeva V.A., Pigarova E.A., Rozhinskaya L.Y.</copyright-holder><license license-type="creative-commons-attribution" xlink:href="https://creativecommons.org/licenses/by/4.0/" xlink:type="simple"><license-p>This work is licensed under a Creative Commons Attribution 4.0 License.</license-p></license></permissions><self-uri xlink:href="https://www.osteo-endojournals.ru/jour/article/view/10209">https://www.osteo-endojournals.ru/jour/article/view/10209</self-uri><abstract><sec><title>Обоснование</title><p>Обоснование: Отсутствие единого подхода в лечении недостаточности и дефицита витамина D явилось стимулом для детального изучения динамики показателей фосфорно-кальциевого обмена, паратгормона, 25(ОН)D (кальцидиола) при применении однократной дозы насыщения колекальциферолом с точки зрения оценки комплексных фармакокинетических свойств.</p></sec><sec><title>Цель</title><p>Цель: Оценить фармакокинетические свойства колекальциферола в дозировке 150 000 МЕ, с позиции эффективности и безопасности его применения в клинической практике.</p></sec><sec><title>Методы</title><p>Методы: Обсервационное, одноцентровое, проспективное, выборочное, неконтролируемое исследование комплексной оценки фармакокинетических свойств однократной болюсной дозы 150 000 МЕ колекальциферола. Для оценки фармакокинетических свойств колекальциферола в дозировке 150 000 МЕ нами были заданы критерии эффективности и безопасности. Критерием эффективности лечения было достижение адекватного уровня витамина D (более 30 нг/мл при исходном недостаточном уровне и более 20 нг/мл для пациентов с дефицитом витамина D). Критериями безопасности коррекции дефицита или недостаточности витамина D было отсутствие жалоб пациентов, нежелательных явлений и/или серьезных нежелательных явлений, а также сохранение основных лабораторных параметров фосфорно-кальциевого метаболизма в пределах референсных значений.</p></sec><sec><title>Результаты</title><p>Результаты: При исследовании эффективности дозы в 150 000 МЕ у пациентов с дефицитом и недостаточностью витамина D установлено, что уровень кальцидиола достоверно выше в группе после лечения колекальциферолом по сравнению с группой до лечения (p&lt;0,05). Пик максимального значения для больных с дефицитом установился на 14 сутки от момента приема колекальциферола и составил 37,1±6,28 нг/мл, а для пациентов с исходной недостаточностью витамина D 40,1±3,71 нг/мл. Начиная с 3 дня от момента инициации терапии установлено постепенное почти линейное увеличение концентрации 25(ОН)D, при этом его значение оставалось в пределах референсных значений на протяжении всего периода лечения. При исследовании безопасности колекальциферола в дозе 150 000 МЕ не выявлено статистически значимых различий в лабораторных параметрах фосфорно-кальциевого метаболизма, как в группе до лечения, так и после коррекции дефицита и недостаточности витамина D в обеих группах.</p></sec><sec><title>Заключение</title><p>Заключение: Колекальциферол в виде однократной болюсной дозы 150 000 МЕ продемонстрировал свою эффективность и безопасность в реальной клинической практике.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: The lack of a unified approach to the treatment of deficiency and vitamin D deficiency stimulated a detailed study of the dynamics of indicators of phosphorus-calcium metabolism, parathyroid hormone, 25(OH)D (calcidiol).</p></sec><sec><title>Aim</title><p>Aim: To evaluate the pharmacokinetic properties of colecalciferol at a dosage of 150 000 IU, from the standpoint of its efficacy and safety in clinical practice.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: Observational, single-center, prospective, selective, uncontrolled study of a comprehensive assessment of the pharmacokinetic properties of a single saturating dose of 150 000 IU of colecalciferol. To assess the pharmacokinetic properties of colecalciferol at a dosage of 150 000 IU, we set efficacy and safety criteria. The criterion for the effectiveness of treatment was to achieve an adequate level of vitamin D (more than 30 ng / ml at the initial insufficient level and more than 20 ng / ml for patients with vitamin D deficiency). The safety criteria for the correction of vitamin D deficiency or deficiency were the absence of patient complaints, adverse events and / or serious adverse events, as well as the preservation of the main laboratory parameters of phosphorus-calcium metabolism within the reference values.</p></sec><sec><title>Results</title><p>Results: When studying the efficacy of a dose of 150 000 IU in patients with vitamin D deficiency and insufficiency, it was found that the level of calcidiol was significantly higher in the group after treatment with colecalciferol compared with the group before treatment (p &lt;0.05). The peak of the maximum value for patients with deficiency was established on the 14 day from the moment of administration of colecalciferol and was 37.1 ± 6.28 ng / ml, and for patients with initial vitamin D deficiency 40.1 ± 3.71 ng / ml.
