Комплексный подход к лечению, включающий операцию, полихимиотерапию и лучевую терапию, существенно увеличивает выживаемость пациентов. Однако это неизбежно приводит к развитию различных отдаленных последствий со стороны эндокринной системы, в том числе оказывает негативное влияние на костную ткань. Снижение скорости линейного роста, низкий конечный рост, недостаточный набор пиковой костной массы – хорошо известные отдаленные последствия лечения. В обзоре на основании данных российских и зарубежных исследователей, а также собственного опыта представлены эндокринные последствия терапии злокачественных опухолей центральной нервной системы в детском и подростковом возрасте, обобщены факторы риска развития различных нарушений со стороны эндокринной системы, проанализированы многочисленные причины, влияющие на рост и развитие кости и приводящие к снижению минеральной плотности костной ткани (МПКТ). Отдельное внимание в обзоре уделяется оценке МПКТ в группе пациентов, перенесших онкологическое заболевание в детском и подростковом возрасте и химиолучевую терапию, обсуждаются вопросы диагностики, профилактики и лечения системного остеопороза у детей.</p></abstract><trans-abstract xml:lang="en"><p>Medulloblastoma is the most common childhood malignant tumor of the posterior cranial fossa. A comprehensive treatment approach, including surgery, polychemotherapy and radiation therapy, inevitably leads to the development of various long-term consequences from the endocrine system, including a negative impact on bone tissue. Reduced linear growth rate, low adult height and low peak bone mass are well-known long-term effects of treatment. Based on data from Russian and foreign researchers, as well as our own experience, this review presents the endocrine consequences of treatment of malignant tumors of the central nervous system in childhood and adolescence, and summarizes the risk factors for various endocrine system disorders, including a decrease in bone mineral density (BMD).
