Профилактика ОП предполагает предупреждение повторных переломов, для чего создаются специальные службы, принципы и эффективность работы которых требуют совершенствования и дополнительного изучения.</p></sec><sec><title>Цель</title><p>Цель. Осветить посредством анализа профильных научных публикаций и собственного клинического материала ключевую роль служб профилактики повторных переломов для снижения заболеваемости остеопорозными переломами.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Клинический материал собран на базе Медицинского центра г. Арташат, Республика Армения. В исследование вошли 2332 пациента в возрасте от 50 до 97 лет: 1656 женщин и 676 мужчин с характерными ОП костей конечностей. В 1-ю группу вошли 765 пострадавших, у которых в 2011–2013 гг. антиостеопорозная фармакотерапия (АФ) назначалась на основании известных рентгенологических методик. Во 2-й группе у 1567 пациентов в 2014–2017 гг. АФ назначали с использованием электронного калькулятора FRAX. В двух клинических группах сравнивали доли пациентов, получивших и не получивших назначение АФ, а также доли пострадавших, у которых были зарегистрированы повторные малоэнергетические переломы. Был также проведен анализ научных публикаций за последние 20 лет по проблеме предотвращения повторных ОП и о роли специальных служб профилактики повторных переломов (СППП).</p></sec><sec><title>Результаты</title><p>Результаты. Сравнительный анализ показал, что во 2-й клинической группе доля пациентов, получивших назначение АФ с использованием калькулятора FRAX, достоверно (p&lt;0,05) возросла на 16,6%, а доля пострадавших с повторными ОП достоверно (p&lt;0,05) снизилась на 6,5% (или в 1,17 раза) по сравнению с 1-й группой. Анализ профильных научных публикаций подтвердил высокую эффективность работы СППП и их важную роль в снижении заболеваемости ОП в целом.</p></sec><sec><title>Заключение</title><p>Заключение. Использование калькулятора FRAX для назначения профильным пациентам АФ позволило достоверно (Р&lt;0,05) повысить на 16,6% долю тех, кому она назначалась, что привело к достоверному (Р&lt;0,05) снижению на 6,5% доли пострадавших с повторными малоэнергетическими переломами. Анализ профильных научных публикаций и собственного клинического материала подтвердил ключевую роль СППП для снижения распространенности ОП.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Fragility fractures (FF) determine main severity of medical and social consequences of osteoporosis. FF prophylaxis considers to prevent repeated fractures. For this reason special services are being created. Their principles and effectiveness require improvement and additional study.</p></sec><sec><title>Aim</title><p>Aim. Тo highlight key role of Fracture Liaison Services (FLS) for reducing incidence of FF by means of scientific publications and own material analysis.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Clinical material was collected at the Artashat Medical Center (Republic of Armenia). Study included 2,332 patients aged 50–97 years, including 1,656 women (71%) and 676 men (29%) with typical FF of limb bones. First group included 765 patients treated in 2011–2013, for whom anti-osteoporotic pharmacotherapy (AP) was prescribed due to known X-ray techniques. In second group of 1567 patients the AP was prescribed based on electronic FRAX calculator in 2014–2017. Proportions of patients in two clinical groups with and without AP were compares along with proportions who had repeated low-energy bone fractures. Moreover, past 20 years scientific publications analysis dedicated to prevention of FF and role of FLS was carried out.</p></sec><sec><title>Results</title><p>Results. Comparative analysis revealed that patients proportion with prescribed AF based on use of FRAX calculator in second clinical group, significantly (p&lt;0.05) increased by 16.6%, while patients proportion with repeated FF significantly (p&lt;0.05) decreased by 6.5% (or 1.17 times) in relation to first group. Analysis of publications has shown high efficiency of FLS and their important role in reducing incidence of FF in general.</p></sec><sec><title>Conclusion</title><p>Conclusion. Use of FRAX calculator for AP prescription allowed to achieve significant (p&lt;0.05) increase by 16.6% in proportion with prescribed specific AP, which led to significant (p&lt;0.05) decrease of patient proportion (6.5%) with repeated low-energy fractures.
