Однако более половины случаев низкоэнергетических переломов происходят у пациентов с МПК в зоне остеопении или даже нормы. Причина в том, что, помимо массы костной ткани, имеет значение и ее микроархитектура, которая не оценивается при стандартной денситометрии. Трабекулярный костный индекс — это неинвазивный, получаемый косвенным путем показатель, характеризующий состояние трабекулярной (губчатой) костной ткани. Он определяется при текстурном анализе изображения, полученного при проведении стандартной денситометрии поясничного отдела позвоночника в прямой проекции. Высокие значения соответствуют хорошо организованной, прочной трабекулярной структуре кости, тогда как низкие значения свидетельствуют о выраженной деградации микроархитектуры. Трабекулярный костный индекс служит самостоятельным предиктором снижения прочности кости, а в сочетании МПК позволяет более точно прогнозировать вероятность перелома, чем каждая из этих величин отдельно. Индекс позволяет уточнить риск как при первичном, так и при вторичном остеопорозе, и в клинической практике применяется как дополнительный параметр при оценке риска остеопороза и переломов.</p><p>На основе литературных данных и собственного опыта эксперты Российской ассоциации по остеопорозу сформулировали рекомендации по использованию трабекулярного костного индекса в клинической практике для более точного определения индивидуального риска перелома в рамках современных взглядов на персонализированную медицину.</p></abstract><trans-abstract xml:lang="en"><p>Traditionally, the assessment of fracture risk is based on measuring bone mineral density using dual-energy X-ray absorptiometry (densitometry). However, more than half of cases of low-energy fractures occur in patients with osteopenia or even normal BMD. The reason is that in addition to the bone mass, its microarchitecture is also important, which is not evaluated by standard densitometry. The trabecular bone score is a non-invasive, indirectly obtained indicator that characterizes the condition of trabecular bone tissue. It is determined by textural analysis of the image obtained during standard DXA of the lumbar spine in a direct projection. High values correspond to a well-organized, robust trabecular bone structure, while low values indicate a pronounced degradation of the microarchitecture. The trabecular bone score serves as an independent predictor of a decrease in bone strength, and combined with bone mineral density, it allows to more accurately predict fracture risk than each of these values separately. The score makes it possible to clarify the risk of both primary and secondary osteoporosis, and in clinical practice it is used as an additional parameter in assessing the risk of osteoporosis and fractures.</p><p>Based on the literature data and their own experience, the experts of the Russian Association on Osteoporosis have formulated recommendations on the use of the trabecular bone score in clinical practice to more accurately determine the individual risk of fracture within the framework of personalized medicine.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>остеопороз</kwd><kwd>рентгеновская денситометрия</kwd><kwd>трабекулярный костный индекс</kwd></kwd-group><kwd-group xml:lang="en"><kwd>osteoporosis</kwd><kwd>DXA</kwd><kwd>Trabecular Bone Score</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Marshall D, Johnell O, Wedel H.
