Провести сравнительный анализ влияния золедроновой кислоты и ибандроната на костный метаболизм у постменопаузальных женщин с остеопорозом. Методология. Открытое проспективное исследование. Материал исследования. Женщины с постменопаузальным остеопорозом, получающие терапию золедроновой кислотой (n=225) и ибандронатом (n=64). Методы исследования. Оценка соматического статуса, лабораторные и инструментальные исследования (клинический анализ крови, биохимический анализ крови, общий анализ мочи, биохимические маркеры костного ремоделирования (БМКР) сыворотки крови: B-CrossLaps — маркер резорбции, остеокальцин- маркер костеобразования, ЭКГ и консультация терапевта по необходимости, остеоденситометрия (двухэнергетическая рентгеновская абсорбциоме-трия) Lj—L                  4 и Neck). Результаты исследования. Проведено проспективное исследование в течение 12 месяцев по изучению влияния азотсодержащих бисфосфонатов на костный метаболизм. Изучена динамика биохимических маркеров костного ремоделирования (БМКР) на фоне терапии золедроновой кислотой и ибандронатом. Получено достоверное снижение как маркеров резорбции (B-Crosslaps), так и формирования костной ткани (остеокальцин). Показатель резорбции костной ткани снижался раньше (уже через 1 месяц) и в большей степени, чем показатель формирования. Полученный характер изменения БМКР на фоне антирезорбтивной терапии обеспечил положительный баланс костного обмена и увеличение минеральной плотности костной ткани. Заключение. Золедроновая кислота и ибандронат оказывали быстрый и мощный эффект на костный метаболизм. Преимущественное снижение маркеров резорбции достоверно обеспечивало положительную динамику МПК. Результаты исследования подтвердили возможность использования БМКР для ранней диагностики (через 1 месяц) реакции на антирезорбтивную терапию и определения индивидуального ответа пациентов на проводимую терапию задолго до изменения МПК. На фоне терапии золедроновой кислотой отмечен наибольший прирост МПК в сравнении с терапией ибандронатом (р&lt;0,05).</p></abstract><trans-abstract xml:lang="en"><p>In open prospective study we determine the efficacy of zoledronic and ibandronic acids on bone metabolism markers and bone mineral density in women with postmenopausal osteoporosis. 225 women with postmenopausal osteoporosis treated with zoledronic acid in dose 5 mg, intravenously, once a year and 64 women treated ibandronic acid were followed up. The investigators assessed clinical and medical history data, evaluated the gynecological and somatic statuses, and made laboratory and instrumental studies: clinical, biochemical blood tests, general urinalysis, determination of serum bone metabolism markers (B-Crosslaps, osteocalcin), osteodensitometry and electrocardiography, therapist ’s examination if needed. Therapy with zoledronic acid 5 mg once a year and ibandronic acid 150 mg once a month was followed by a significant reduction in resorption markers (B-Crosslaps) just following one month. Formation markers decreased to a lesser degree, which was indicative of preserved positive bone remodeling balance during the therapy. Determination of biochemical markers of bone remodeling permits assessment of an individual patient response to antiresorbtive therapy just in the first month after infusion.
